By checking, I agree and acknowledge that the Community Room requested is located in my building.

(Ex: If you live in UHV 1 - 4609 N Graduate, you are requesting to reserve the Community Room in the building you reside in)

I have read and agree to the following Community Room Rules: 

1. Persons under eighteen (18) must be accompanied by a parent or Guardian.

2. Residents must accompany their guests at all times.

3. Absolutely No Smoking is allowed in the community room.

4. Community Rooms are under twenty-four (24) hour recorded video surveillance.

5. If equipment or appliances are not working properly, please report the problem to the office as soon as

possible.

6. The resident will be responsible for cleaning the room after their function. Please leave the 

room clean and free of garbage. If the community room is not cleaned properly, the resident 

who reserved the room will be charged a cleaning fee.

7. Reservations must be made at least 48 hours in advance, Monday through Friday, and no later than 3:00 PM on Fridays.

These rooms are for your entertainment and enjoyment. Please 

use care and caution when using the facility and operating the equipment. Any damages incurred while 

using the Community Rooms will be charged to the resident who reserved the room.
